Nevada Workers' Compensation Forms and Deadlines

Three forms people often confuse

Nevada workers' compensation uses separate forms for employee notice, medical treatment reporting, and employer reporting. They serve different purposes and are filed through different parties. The summaries below reflect language in official DIR/WCS publications—not instructions from Ruiz Law Group.

Form C-4

Employee's Claim for Compensation / Report of Initial Treatment

Completed with the treating medical provider

  • Documents initial treatment related to the work injury
  • Official materials discuss a 90-day period to seek treatment after notice
  • The treating provider has filing duties described in official WCS materials

Form C-3

Employer's Report of Injury or Occupational Disease

Employer and insurer reporting process

  • Filed by the employer or insurer, not by the injured worker through this page
  • Part of the employer's reporting obligations under Nevada workers' compensation
  • Workers may receive copies or status updates from the employer or insurer

Form C-1

Notice of Injury or Occupational Disease

The C-1 is the employee's written notice that a work injury or occupational disease occurred. Official Nevada materials describe giving notice to the employer as soon as practicable and no later than 7 days after the incident or after the employee knew or should have known that the condition was work-related.

This notice is separate from a medical claim form. Providing C-1 notice does not by itself complete treatment reporting or employer filing steps described in other official forms.

Form C-4

Employee's Claim for Compensation / Report of Initial Treatment

The C-4 is completed in connection with medical treatment for a work injury. Official WCS materials discuss seeking treatment within 90 days and describe provider responsibilities for filing the form. Workers typically complete their portion with the treating provider rather than through this educational page.

Form C-3

Employer's Report of Injury or Occupational Disease

The C-3 is part of the employer and insurer reporting process after a work injury or occupational disease. It is not something this page files on a worker's behalf. Employers and insurers use it within the administrative system described in official DIR/WCS materials.

If you are an injured worker, your practical step is usually to give timely notice (C-1), seek appropriate treatment (C-4 with a provider), and keep copies of paperwork the employer or insurer provides. Confirm employer-specific reporting instructions with the employer or on official WCS pages.

Frequently asked administrative questions

Is the C-1 the same as the C-4?

No. The C-1 is employee written notice to the employer that an injury or occupational disease occurred. The C-4 is the Employee's Claim for Compensation / Report of Initial Treatment completed in connection with medical care. Official materials treat them as separate steps with different purposes and timeframes.
